CCSP
In-Home Support Services in Georgia to Assist Frail, Elderly Individuals Remain Living in Their Homes
CCSPÂ is a Medicaid waiver program designed to help frail older adults remain living in their own homes or communities and to receive services in those locations as an alternative to nursing homes. This means persons who qualify for the CCSP program may receive home care or skilled nursing care services at no charge or a small monthly cost-share as Real McCoy Home Care is reimbursed directly by the State of Georgia.
Who is Eligible For CCSP Program
- A person must be 65 years of age or older with a chronic medical condition
- or under 65 years of age and disabled.
- Require nursing home level care
- Receiving SSI Benefits
Common Medical Conditions To Qualify Includes:
- Asthma or other pulmonary problems
- Diabetes
- Arthritis (rheumatoid and osteoporosis)
- Stroke
- High Blood Pressure
- Amputations
- Alzheimer’s Disease
- Spinal Cord Injury
CCSP Eligibility Requirements
- Functional impairment caused by physical limitations and have an unmet need for care
- NOTE: Alzheimer’s and dementia are physical conditions.
- Approval of an intermediate level of care (LOC) certification for nursing home placement
- One’s ability/inability to independently complete their ADLs (Activities Of Daily Living)
- Approval of care plan by client’s physician
- Health and safety needs can be met by CCSP
- Participation in one waiver program at a time
- A client is NOT required to be homebound to receive CCSP services

SOURCE is an enhanced primary care case management program serving frail elderly and disabled beneficiaries. The program works to improve the health outcomes of persons with chronic health conditions by integrating primary medical care with home care services through case managers.
Who is Eligible For SOURCE Program
- Receive SSI/Medicaid (Supplemental Security Income)
- A person must be 65 years of age or older with a chronic medical condition
- Require nursing home level care
